File A Claim

Pursuant to the Court's Order Granting Preliminary Approval Pursuant to Fed. R. Civ. P. 23(e)(1) and Permitting Notice to the Class, dated February 3, 2022, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release ("Claim Form") in accordance with the instructions contained therein. To obtain a copy of the Claim Form, click here. The deadline to submit a timely Claim Form is May 25, 2022.

You may submit your Claim Form using any one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Claim Form will include a certification under penalty of perjury that your Claim Form is accurate and complete.

Whichever submission method you choose, you are required to provide documentation to support your claimed transactions. Please remember to provide supporting documentation of all transactions listed on your Claim Form. In the event that you submit your Claim Form online, you must retain confirmation of such filing.

If you fail to execute and submit a timely, properly completed and addressed Claim Form, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.

To submit a Claim Form by mail, complete and sign the Claim Form in its entirety, with all required supporting documentation and mail it, postmarked no later than May 25, 2022, to the Claims Administrator at:
